General Information

It is an outpatient psychiatric office providing mental health and counseling services to patients from age 6 to 70 years old. We provide psychiatric evaluation, medication management, individual psychotherapy and groups.
Yes, We accept most private insurance plans as well as Medicare, Medicaid, and private company plans. Our goal is to help you access healthcare and maintain wellness regardless of your insurance or ability to pay. If you have financial concerns about receiving services, please ask to see a Patient Services Representative.
You can login to your Patient Portal by desktop, laptop and/or phone called the IMS Care Portal.
Yes, there will be an administrator cover charge of $5.00 and $1.00 per page fee. However, we can fax your medical records to another providers without charge.
Yes, we can fax your medical records to your provider, lawyer and therapist without charge, with your sign Release of Information Consent Form. You can call our office to obtain a consent form or send a signed Release of Information Consent Form from the requesting party.

name, their date of birth, their reason for the visit, who referred the patient and insurance information.
What do you need to bring to my appointment?

You will need:

  • The New Patient Welcome Packet completed
  • Patient’s insurance card
  • Legal guardianship papers (If applicable)
  • Bring all prior psychiatric/psychological evaluations, school and medical reports
  • Bring all patient’s prescription bottles
  • Patient’s driver's license/ID card
  • Parent’s/guardian’s ID card (If patient is under 18 year old)
The cost will depend on your insurance plan benefits. It will be very helpful to you, if you ask your insurance plan what is your deductible and co-pay for seeing a mental health provider. All payments are due at the time of your visit. We accept cash, HRA/HSA, and credit cards.
Your initial visit, with your physician, physician assistant or nurse practitioner will be approximately an hour. Your follow-up visit is approximately 15-20 minutes.
Your therapy sessions are approximately 45-60 minutes.
